Multidisciplinary Long-Term Recovery
Lancaster General Health has established their multidisciplinary Post-COVID-19 Recovery and Rehabilitation Therapy program to treat patients experiencing post-COVID-19 issues in a coordinated manner. The goal of the program is to provide consultation, direct care as needed, and on-going care through primary care offices for patients who have lingering physical, cognitive or behavioral symptoms following COVID-19 infection.
The Lancaster General Health team features members from case management, physical therapy, occupational therapy, speech pathology and neuropsychology. The goal of the program is to evaluate each patient individually and provide a customized care plan. Patients will be evaluated initially by our physical therapy and occupational therapy team.
Based upon evaluation, screening tools and outcome measures, patients may be referred to additional resources such as neuropsychology practice (ages 12 and up), behavioral health or other providers. For patients identified with high complexity, complications or impaired progress, the team will recommend referral to a physiatry specialist for medical oversight including collaboration and referrals to cardiology or pulmonology services as needed. A rehab case manager can be assigned as a personal navigator for patients as required.
Patients are eligible for treatment if they have recovered from COVID-19 and are experiencing lingering health issues including:
Weakness
Fatigue
Pain
Difficulty performing usual activities
Emotional issues
Cognitive Impairment
Psychological Issues: PTSD, insomnia, anxiety and depression

Physician Referrals: Referring providers can make a referral to the COVID-19 recovery program (use AMB LGH Physical Therapy services or COVID-19 Recovery Panel) through their normal Lancaster General Health referral process, or call 717-544-3547.
Self-Referrals: If you are a patient experiencing lingering issues following a COVID-19 infection, you can ask your primary care doctor for a referral or complete this form and you will be contacted.

Main Line Health Post-COVID Recovery Program
Multidisciplinary Long-Term Recovery
The Main Line Health Post-COVID Recovery Program will provide health services tailored to each patient's individual needs. The Program’s multidisciplinary team includes physiatrists, neuropsychologists, psychologists, medical specialists, physical therapists, occupational therapists, speech language pathologists and case managers. If the patient does not have a specialist, the program is able to make referrals to cardiology, pulmonology, dermatology, gastroenterology, infectious disease, primary care and ENT. The Recovery Program communicates with and works closely with the patient's primary care team to create a successful care plan.
The Post-COVID Recovery Program includes:
Initial evaluation: A patient may be referred to the program directly by their primary care physician or specialist provider or they may require an evaluation by a physiatrist(doctor of physical medicine) to assess their overall needs. When the patient calls the main number, a nurse case manager and/or a nurse practitioner will collect intake information and assist in triaging to the appropriate care.
Individualized therapies:
Physical therapy will conduct evaluations and design a treatment plan to address deficits in strength, balance, issues related to vertigo, endurance, joint pain, headaches, neurological dysfunction, and fatigue. As appropriate, a comprehensive exercise program will be developed to progress the patient at a pace that is individualized and appropriate to their condition.
An occupational therapist will work with patients having difficulty with activities of daily living, including eating, bathing, dressing and toileting and help patients return to household, occupational, and leisure activities.
A speech language pathologist will assist with any swallowing and cognitive impairments including brain fog, short-term memory loss, word finding problems, voice control and reduced concentration
Behavioral health services will conduct a neuropsychological evaluation for neurocognitive impairments and affective symptoms that may impact recovery. Individual and group psychology sessions will be offered to address behavioral health needs and psychological impact of COVID-19 on recovery.
Specialists will treat the patient for the condition that they were referred ensuring that care is well coordinated among all team members.
Fitness programming will be offered, when appropriate, to help participants maximize their level of physical fitness for the long-term. Under the constant supervision a nationally accredited personal trainer, participants will progress through a structured 8-week exercise program designed to help those recovering from COVID-19 optimize their quality of life through increased flexibility, strength and endurance.
Both virtual visits and in-person outpatient rehabilitation are available.
Patients who have had a positive laboratory diagnosis of COVID-19 and continue to experience health issues at least 30 days following the positive test results are eligible for this program.

Multidisciplinary Long-Term Recovery
Penn Medicine has opened the Post-COVID Recovery Clinic to provide multidisciplinary assessment and resources for patients recovering from COVID-19, with a principal objective for post-COVID patients re-entering the community to ensure continuity of care, a known critical factor in the prevention of mortality and morbidity in long-term disease.The clinic will also serve to create a registry to accurately track important clinical patient data for a disease about which little is known.
Referrals are welcome for all recovering COVID patients who are experiencing lingering health issues including: Autonomic function: Pain, Hypercoagulability, Poor endurance, Impaired renal function, CI Myopathy / Polyneuropathy, Residual pulmonary manifestations, Psychological issues: PTSD, Insomnia, Cognitive impairment, Myocardial Injury, Progression of chronic issues.

Neurological Long-Term Recovery
The Penn Medicine Post-COVID-19 Neurological Care Clinic aims to assess and treat long-haul COVID patients suffering from neurological symptoms. Recently launched by the Department of Neurology at the University of Pennsylvania’s Perelman School of Medicine, the clinic focuses on patients who previously tested positive for COVID-19 and experience symptoms related to cognition, headache, vertigo, and brain fog.
The Clinic is conducting visits both in-person and virtually. All neurological patients are being referred through Penn Medicine’s Physical Medicine & Rehabilitation Post-COVID Assessment and Recovery Clinic.

Temple’s Post-COVID Recovery Clinic is here to help COVID-19 long-haulers recover and improve their quality of life. Managed by the experts at the Temple Lung Center, the Clinic brings together leading specialists in lung disease to address the full breadth of COVID-19 symptoms and needs, using the most up-to-date treatments available. The Recovery Clinic will stay with you as long as you need them, track your progress over time to identify and treat any new issues that may emerge, and provide integrated referrals and consultations for any additional services you may need.
The Clinic provides a full array of treatment options, including:
Pulmonary rehabilitation — helps to manage symptoms while you recover. Pulmonary rehabilitation may include counseling on diet and exercise, mental health support, and helpful breathing techniques.
Supplemental oxygen therapy — helps you get the oxygen needed to go about your daily life with less fatigue or difficulty.
Medications — may be used to reduce inflammation in the lungs or reduce scarring that can develop after a COVID-19 infection.
More advanced treatments and procedures — in rare cases, patients whose symptoms are especially serious and long-lasting may require more advanced treatments such as lung transplant.

Multidisciplinary Long-Term Recovery
The UPMC is dedicated to supporting and treating patients who have recovered from initial COVID-19 infection but are still experiencing persistent symptoms through their Post-COVID Recovery Clinic. The Clinic will see any patients who are experiencing the following symptoms: Dyspnea (difficult or labored breathing), Shortness of breath, Exercise intolerance, Fatigue, Weakness, Persistent cough, Autonomic instability or Cognitive difficulties.
UPMC will provide an initial telemedicine visit (or an in-person visit for more severe cases) for qualifying patients to perform a clinical assessment. The Clinic will then perform individualized evaluation and testing to identify any additional health complications.
Based on the results of the assessment, evaluation and testing, each patient will be referred to the appropriate specialists / services and specialized care that UPMC has to offer: Pulmonary medicine, Cardiology, Infectious diseases, Nephrology, Physiatry, Physical therapy, Primary Care Occupational therapy, Radiology, Neuropsychiatry, Behavioral health, Social work, and Physical medicine and rehabilitation.

Multidisciplinary Long-Term Recovery
The AHN Post-COVID-19 Recovery Clinic connects post-COVID patients to clinicians who can diagnose symptoms, develop personalized treatment plans, and ease their concerns. The Recovery Clinic provides the following:
Integrated team: No two patients are the same, which is why we need a comprehensive care team standing by to treat any and all symptoms. Currently, the clinic engages clinicians from pulmonology, cardiology, integrative medicine, family medicine, behavioral health, critical care, and neurology. Together, the team helps patients manage and overcome post-COVID-19 complications.
Dedicated nurse navigator: Finding a path to care can be complicated. That’s why each patient is assigned a nurse navigator to help manage treatment paths and answer questions along the way.
Ongoing research and development: The AHN Research Institute is actively engaged in assessing and analyzing data collected from this clinic. As they see more patients, the Clinic will be able to gather learnings that will inform the treatment plans and long-term care of this patient population.
Care throughout the community: While the clinic is based out of the Wexford Health + Wellness Pavilion, it is available to patients throughout the region, with participating clinicians providing care at Allegheny General, Allegheny Valley, Jefferson, Grove City and Saint Vincent hospitals. The clinic also offers virtual care and in-person appointments.
