PULMONARY Long-Term Recovery

The Albany Medical College Post-COVID Care Clinic is designed to provide care for pulmonary post-COVID health issues. The Clinic is staffed by a team of pulmonologists across the Albany Medical Center.

The Post-COVID Care Clinic will first provide a pulmonary function test for each patient to evaluate the function of their respiratory system. Subsequent tests, referrals and treatment will be tailored to fit each patient’s symptoms and medical history.

The Clinic will treat any adult patient who has previously tested positive for COVID-19 or COVID-19 antibodies, but is no longer acutely ill while still experiencing long-term pulmonary symptoms. NOTE: The Clinic cannot treat patients whose post-COVID symptoms are not pulmonary.

Multidisciplinary Long Covid Recovery Program

The Long Covid Program at New York-Presbyterian provides care and treatment plans for adults who believe they are experiencing long-term symptoms of COVID-19, also known as Long Covid.

NYP’s providers start by listening to your experience. They will create an initial evaluation of your health by asking you questions to help them better understand your symptoms and health experiences. Next, the Program will work with you to develop a plan. It may be as simple as listening to your concerns and providing supportive care. You may also benefit from working with particular specialists. If so, the Program’s patient navigator can help coordinate that care.

Northwell Health Coronavirus Related Outpatient Work Navigators (CROWN) Program

Acute COVID-19 Recovery

For seriously ill COVID-19 patients who have high-risk features, such as persistent shortness of breath or fever, possibly with an underlying health condition, the Northwell Health Coronavirus Related Outpatient Work Navigators (CROWN) program provides a near-hospital level of care. The CROWN program allows patients to be treated effectively without having to go to a hospital.

Developed by Northwell pulmonologist Gita Lisker, MD, the CROWN program can quickly send out a home care nurse, a phlebotomist to draw blood for COVID-specific tests, a pulse oximeter to track the blood-level of oxygen and a tank of supplementary oxygen, all generally on the same day the patient is enrolled in the program. Other services are also available, such as home delivery of anticoagulant medication if the risk of blood clots is high, placement of an intravenous line or an X-ray exam done with a portable machine.

The Mount Sinai Center for Post-COVID Care recognizes that recovery from COVID-19 is posing a new set of challenges for patients and physicians alike and that the virus can affect many different systems within the body—from the lungs to the heart to the kidneys. As such, the Center is bringing together a clinical team from a broad range of specialties to address the issues of COVID-19 aftercare.

Cardiac Long-Term Recovery

The Mount Sinai Post-COVID Cardiac Clinic offers evaluations for individuals who have been previously infected with COVID-19 and have experienced any of the following symptoms: Difficulty breathing at rest or with exertion, Chest pain, Palpitations, Fainting / loss of consciousness, Fatigue Shortness of breath, or Irregular heartbeat. Patients with hypertension, lung disease, former or current smokers, and those who are diabetic qualify for evaluation as well. Patient must be at least 30 days post initial infection.

Experts at the NYU Langone Post-COVID Care Program provide comprehensive evaluation and treatment for people who are experiencing long-term symptoms or other lasting health impacts after COVID-19 infection. Some of the common long-term symptoms people experience after COVID-19 include the following:

The Post-COVID Care Program evaluates and treats people ages 18 and older who are experiencing COVID-19 symptoms or related health problems persisting at least 4 weeks after the initial infection. They do not require proof of a COVID-19 diagnosis to make an appointment with our program.

Your first visit with the Post-COVID Care Program is a video visit with a nurse practitioner, who takes your health history and discusses your symptoms with you. Based on this information, the Program schedules an appointment with the appropriate specialists, who complete a full evaluation to determine the severity and scope of your symptoms and create a treatment plan.

Multidisciplinary Long-Term Recovery

Weill Cornell Medicine takes a holistic approach to care after the ICU through its Post-ICU Recovery Clinic. The Clinic’s expert, critical-care trained physicians, social workers, dietitians, clinical pharmacists and psychologists will work with each patient to evaluate their physical, emotional and cognitive needs. In addition to a thorough medical exam, basic pulmonary and strength testing, they screen for anxiety, depression, sleep disorders and PTSD. They also ensure patients are taking the right medicines, have all the equipment they need at home and are referred to appropriate specialists, including physical therapy, acupuncture and mental health services.

Visits at the Clinic may last up to 2 hours (video visits up to 1 hour ) and may include:

  • Meetings with one or two physicians, and potential meetings with a psychologist, social worker, pharmacist or dietitian

The Kellman Wellness Center is trained in the latest diagnostics and treatment options for long COVID and is now  treating patients in the New York City area struggling with long-haul COVID-19 symptoms.

Dynamic Sports Physical Therapy Post Acute COVID Syndrome Rehabilitation Program

In partnership with Mount Sinai Hospital, Dynamic Sports PT is now treating patients experiencing Post Acute COVID Syndrome (PACS).

The goal of physical therapy for those with PACS is to enable patients to increase their physical activity levels and exercise capacity. This is primarily accomplished by targeting strength, endurance, balance, and functional abilities with a wide scope of movement-based interventions. In addition, extra concern is given to each patient’s response to activity, particularly their respiration—or breathing—while performing these tasks. Therefore, patients’ oxygen saturation—which measures how well they are breathing—is monitored and regulated before, during, and after physical activity to ensure it does not exceed a certain level.

The facility’s PACS rehabilitation protocol is exercise-based and consistent with evidence-based practices for similar conditions. In addition to monitoring respiration, the team assesses the following parameters during a patient’s first visit and at each subsequent appointment:

  1. Exercise and functional capacity

  2. Presence of pre-existing conditions

  3. Muscle and joint range of motion

  4. Strength

  5. Balance

  6. Ability to complete normal activities of daily living

Once these baseline assessments are recorded, your physical therapist will design a personalized treatment program based on your specific symptoms, impairments, and goals. Programs vary from patient to patient, but in most cases the following interventions are used:

  1. Generalized strengthening exercises to build back muscles that may have been weakened by inactivity

  2. Progressive aerobic and cardiovascular endurance exercises to increase activity levels and exercise capacity

  3. Mobility and stretching exercises to improve flexibility and range of motion

  4. Neuromuscular electrical stimulation to assist with muscle strengthening

  5. Biofeedback, a technique that teaches patients to voluntarily control certain bodily processes through electrical sensors and audio/visual cues

  6. Mindfulness-based techniques, which help patients identify and understand triggers that exacerbate their PACS symptoms; when combined with biofeedback, these two techniques can train patients to downregulate these processes and effectively reduce their symptoms

PACS rehabilitation can be performed remotely through a virtual portal, also known as Telehealth. Required equipment for Virtual Physical Therapy would include:

SUNY Downstate Health Sciences University’s University Hospital of Brooklyn has established a dedicated outpatient clinic for the ongoing care of discharged patients following inpatient and outpatient treatment at SUNY Downstate for COVID-19. The Hospital is committed to providing all the care a patient may need during their ongoing recovery by addressing the potential side-effects of medications patients received during their treatment as well as the ongoing physical and mental issues that have been reported following COVID-19.

Each patient with the Clinic receives an initial telemedicine visit to assess their immediate healthcare needs. They will then be scheduled for an in-person visit to the clinic at University Hospital of Brooklyn for tests and any needed follow-up treatment. In addition to providing care for the short and long-term physical effects of COVID-19, the clinic also assesses the mental health status of these patients. It provides access to resources to help them cope with the emotional trauma associated with their diagnosis and treatment. The clinic will also carefully track the incidence of post-COVID-19 health impacts over time, as well as the success of various therapeutic treatments during recovery.

The Jamaica Hospital Medical Center Post-COVID Care Center was formed in response to the growing number of New York City residents who are considered “long-haulers”, individuals who have experienced COVID-19-related symptoms for an extended period of time. The Care Center includes a multidisciplinary team of neurologists, pulmonologists, psychiatrists, primary care physicians and other specialists who are committed to providing personalized patient care.

The physical and rehabilitation medicine specialists at Sunnyview Rehabilitation Hospital, along with their highly-regarded neuropsychology team and physical, occupational, and speech/language therapists, work in close collaboration to address the ongoing side effects that impact the lives of long-haul COVID patients. 

You may be eligible to enroll in the Program if:

  1. You haven't yet returned to your normal state of health, and it's been more than four weeks since you were infected with COVID.

  2. You were infected with COVID and recovered, but now your symptoms have returned or you've developed new symptoms you didn't have before.

Richmond University Medical Center's Post COVID-19 Care Center (PCC Center) is available for those patients who have been treated for COVID-19. In addition to coping with myriad changes to their medication regimens, patients recovering from these conditions often experience long lasting side effects, including Pulmonary disease, Cardiac related issues, Anxiety and depression and Chronic fatigue and loss of stamina.

A specific pediatric program involving similar sub-specialists is offered for children recovering from COVID-19 symptoms and Multisystem Inflammatory Syndrome in Children (MIS-C).

The Post-COVID Recovery Center at Staten Island University Hospital is part of Northwell Health — New York state’s largest health system. The program treats patients who were infected and treated for COVID-19 and who may still feel its effects. Because recovery from COVID-19 can be different for everyone, we are committed to developing a personalized plan of care for each patient. Patients who were treated for COVID-19 typically experience long-lasting side effects in the following areas: Lung, Cognition, Heart, Fatigue and Stamina, and Mental Health.

Patients at the Recovery Center undergo a full diagnostic evaluation by physicians involving a review of previous medical records, bloodwork, symptom-specific evaluations and mental health exams. Based on this evaluation, specialists will develop a personalized plan of care. Multidisciplinary Long-Term Recovery

NYU Langone Huntington Medical Group is providing care and monitoring through its COVID-19 Recovery Care services to aid in the long-term COVID-19 recovery process. The Medical Group’s team ensures that patients receive specialized care and evaluation to support their recovery after COVID-19, whether they recovered at home or needed hospital care.

Care provided includes the following:

  • Video doctor visits within 24 to 48 hours after your return home for people who received hospital care

  • Assessment and monitoring for at-home oxygen use

  • Referrals for rehabilitation care, including cardiac rehabilitation, physical therapy, and occupational therapy

  • Follow-up imaging services and diagnostic testing to monitor your overall health

The purpose of the Stony Brook Medicine Post-COVID Clinic is to help COVID-19 survivors who continue to experience lingering complications of the virus. Because the virus can affect individuals in different and complex ways, patients at the Post-COVID Clinic will have access to a range of clinical specialists such as cardiologists, neurologists and lung, kidney, and vascular specialists, as well as psychiatrists and psychologists.

SPEAR Physical Therapy is a partner of Mount Sinai and have therapists who are specially trained to assess and treat post acute COVID-19 symptoms (Long COVID or PACS (Post Acute Covid Syndrome). 

The respiratory health of our community has become increasingly more important. Our team of therapists is committed to helping those affected by COVID-19, as well as those affected by other pulmonary conditions, regain their mobility, decrease pain, and improve lung function for their overall health.

We are offering lung strengthening breathing exercises and endurance training in-person and through our online Telehealth platform, SPEAR LIVE.

St. Charles Hospital Post-COVID Outpatient Rehabilitation Program, Catholic Health Services

Rehabilitation and Therapy

The St. Charles Hospital Post-COVID Outpatient Rehabilitation Program offers medically-supervised physical therapy for endurance training, strength training and education. Group sessions in the COVID-19 rehabilitation program are held twice weekly over a period of four weeks. Oxygen, blood pressure and heart rate levels will be monitored before, during and after exercising. Individual sessions are available with frequency and duration of the sessions to be determined based on an initial evaluation.

Patients who are asymptomatic and at least 14 days past a COVID-19 diagnosis are eligible to enroll in the program.

Multidisciplinary Long-Term Recovery

WMCHealth is offering Post-COVID-19 care for ongoing recovery for patients in the tri-state area who continue to deal with issues surrounding their physical, mental and emotional health and quality of life. Patients with lingering symptoms after a known or suspected COVID-19 infection may make an appointment.

At your appointment with the Post-COVID-19 Recovery Program, a primary care doctor will give you a physical exam and review your COVID-19 history. Then, a clinical patient navigator will be assigned to guide you through a treatment plan tailored to your specific needs for specialized care. The program provides patients with access to Westchester Medical Center physicians with expertise in primary care, pulmonology (lungs), cardiology (heart), nephrology (kidneys), psychiatry (mental and emotional health), radiology, and other specialties as needed.

WMCHealth physicians who evaluate you will follow up with your regular doctor to coordinate your ongoing treatment and recovery plan. If you don’t already have a primary care doctor, they will help you establish an ongoing relationship with one of their providers.

The St. John’s Riverside Hospital Post-COVID Care Program provides specialized services, including Diagnostic, Testing, Referral, and Management Services to the thousands of recovered COVID-19 patients in the Southern Westchester area. The Program works in conjunction with the primary care provider of each patient to ensure appropriate medical care.
