The Deborah Heart and Lung Center Post-COVID Recovery Program offers treatment services for those who continue to have health problems well after recovering from their initial bout of the COVID-19 virus. The Post-COVID Recovery Program also offers treatment for patients who have brand new symptoms and medical conditions triggered by COVID-19.
Patients in the Recovery Program receive a thorough evaluation, followed by a diagnosis and individualized treatment plan. This plan will address either the continuing health problems of those who never fully recovered from their initial bout of the virus; or for the brand-new, previously undiagnosed, medical problems which have emerged post-COVID. The program will integrate other specialties in the treatment plan of the patient as deemed necessary, including cardiology, hematology, radiology, pulmonary rehab, and physical therapy.
Patients must have:
Previously tested positive for COVID-19 and were not hospitalized, but who continue to experience respiratory symptoms more than six weeks following infection or
Previous hospitalization for treatment of COVID-19 or
A pre-existing underlying lung disease and who have had COVID , and
Currently testing COVID- negative for an active infection

Multidisciplinary Long-Term Recovery / Rehabilitation and Therapy
The Atlantic Health System has developed a COVID Recovery Center in two of their New Jersey locations to address the need to treat the longer-term effects of COVID-19. The Recovery Centers will provide rehabilitation services to address a wide range of COVID-19 related symptoms throughout the body and will receive support from cardiologists, pulmonologists, behavioral health specialists, neurologists, rehabilitation and physical therapists, and primary care physicians.
Patients at the Recovery Center will first speak with a designated care coordinator who will evaluate patient symptoms and provide specialist referrals. The Atlantic COVID Recovery Center is committed to providing aftercare for any individuals still experiencing COVID-19 symptoms 30 days after their initial diagnosis.

Penn Medicine has opened the Post-COVID Assessment & Recovery Clinic to provide multidisciplinary assessment and resources for patients recovering from COVID-19, with a principal objective for post-COVID patients re-entering the community to ensure continuity of care, a known critical factor in the prevention of mortality and morbidity in long-term disease.The clinic will also serve to create a registry to accurately track important clinical patient data.
Referrals are welcome for all recovering COVID patients who are experiencing lingering health issues including: Autonomic function: Pain, Hypercoagulability, Poor endurance, Impaired renal function, CI Myopathy / Polyneuropathy, Residual pulmonary manifestations, Psychological issues: PTSD, Insomnia, Cognitive impairment, Myocardial Injury, and Progression of chronic issues.

The expert team of Respiratory and Physical Therapists affiliated with Advantage Therapy Centers has been treating pulmonary patients with breathing disorders for over 20 years. Now they are using their experience to provide rehabilitation for patients recovering from the respiratory complications and deconditioning caused by COVID-19.
After the virus has left their system, patients can continue to experience shortness of breath, physical impairment, fatigue, and difficulty accomplishing daily tasks. Some even struggle with pulmonary complications such as scarring of lung tissue. Advantage Therapy Centers are the only Comprehensive Outpatient Rehabilitation Facilities in South Jersey authorized by Medicare to provide Pulmonary Rehabilitation. The program is open to anyone who may be experiencing symptoms beyond four weeks of being diagnosed with COVID-19. Even people who did not have COVID-19 symptoms in the days or weeks after they were infected can have post-COVID conditions. This includes those who may have been unable to receive definitive results through a positive antibody or antigen test.

Multidisciplinary Long-Term Recovery
Hackensack Meridian Health's COVID Recovery Center is New Jersey’s first and only center dedicated to assisting COVID-19 patients after their recovery. Recovered COVID-19 patients who need additional medical care will receive a tele-screening by one of Hackensack Meridian Health’s COVID Recovery Center physicians. Based on the screening results, the physician will create a customized care plan and connect the patient with specialists if needed. Conditions addressed include:
Difficulty breathing, speaking or swallowing, possibly resulting from heart or lung problems
Difficulty walking or performing self-care tasks
Feelings of extreme tiredness and fatigue
Weakness in the arms or legs

The COVID Rehabilitation Program at JFK Johnson Rehabilitation Institute offers an outpatient program that is tailored specifically to the needs of individuals who contracted COVID-19 and are no longer infected, but are experiencing ongoing post-COVID symptoms and side effects of COVID-19 treatment. Most patients participate for 3-6 months, but treatment may continue for a longer period of time depending on each patient’s needs.
JFK Johnson’s Rehabilitation Team includes specialized physical medicine and rehabilitation physicians, as well as highly trained pulmonary, cardiac, neurologic, physical and occupational therapists and nurses.

Pediatric Multidisciplinary Long-Term Recovery
The Hackensack Meridian Children’s Health Pediatric COVID-19 Recovery Center is a multidisciplinary center with the expertise to address symptoms such as fatigue, chest pain, and shortness of breath, “brain fog,” muscle and joint pain, and any other persistent symptoms following an acute COVID-19 infection in children and adolescents to age 22.
To address the wide range of symptoms that some children experience after infection with COVID-19, this special COVID Recovery Center brings together multiple pediatric specialists at a single visit to evaluate a child experiencing “Long COVID”. Pediatricians, pediatric rheumatologists, pediatric pulmonologists, pediatric cardiologists, pediatric neurologists and pediatric rehabilitation specialists work collaboratively to evaluate and offer treatment solutions.
Your child will be seen by specialists tailored to his or her specific symptoms reported on a pre-visit symptom questionnaire. Social work and child life services are available to support the mental health needs and well-being of each child and family.
Eligibility: Any child, under the age of 22 years old, who had COVID-19 or MIS-C, diagnosed either via PCR or antibody can be seen in the clinic—no matter when the child was diagnosed. It is imperative that patients with active COVID infection not make an appointment until 14 days symptom free or make us aware of current status to accommodate virtual visits.

The Monmouth Medical Center Southern Campus [MMCSC] Post-COVID Recovery Program is helping COVID-19 patients with lingering health issues recover fully through coordinated rehabilitation and outpatient services designed to return them to optimal health. The program, which is available to COVID patients whether or not they were previously hospitalized, offers coordinated outpatient rehabilitation, including physical and speech therapy, and access to a continuum of care, such as additional outpatient testing, radiology or prescriptions.

Multidisciplinary Long-Term Recovery
To help people manage what has been called “post-COVID syndrome,” Saint Barnabas Medical Center and RWJBarnabas Health Medical Group have collaborated to create the Post-COVID Comprehensive Assessment, Recovery and Evaluation (CARE) Program. Through the program, a nurse navigator takes each patient through appointments, testing and follow up with a multi-disciplinary team including pulmonologists, cardiologists, and other specialties.
Candidates for the CARE program should have a diagnosis of COVID and still have symptoms at least 4 weeks after diagnosis. Lingering symptoms may include cough, shortness of breath, cognitive complaints including brain fog, GI and fatigue. Currently all appointments are in-person, although the program does have the capability of offering TeleMedicine if needed.

The Kessler Rehabilitation Center Recovery and Reconditioning Program provides physical, cognitive, and psychological support for patients after they have endured long-term hospitalization or significant weakness and deconditioning from a disease like COVID-19. The Recovery and Reconditioning Program offers a team of clinical experts who can assist post-COVID patients in the healing process, addressing sequelae such as fatigue, dyspnea, weakness, headache, dizziness, arthralgia/myalgia, anxiety, sleep disorders, and depression.
The number one goal of the program is to allow patients return to their previous quality of life. The program was designed with input from doctors, nurses, and therapists and follows ACSM guidelines for cardiac monitoring of patients, either in the clinic or through telemedicine.

AtlantiCare has opened the Post-COVID-19 Long-Haul Clinic to provide ongoing care for individuals who have complications from COVID-19. The Clinic is open to those who have been diagnosed with COVID and have experienced persistent symptoms/health issues lasting or appearing more than 30 days after being infected with the virus.
Through the service, which opened on April 26, 2021, an AtlantiCare board-certified internal medicine physician will hold a one-hour telehealth screening and assessment meeting with each patient. The care team identifies treatment/rehabilitation needs, then coordinates virtual and in-person appointments with specialists as needed. The Post-COVID-19 Long-Haul Clinic Team will continue to coordinate care with the patient and the individuals’ primary care provider and specialists.

The Comprehensive COVID-19 Recovery Program at University Hospital follows an integrated model of care that begins with primary care and navigates patients through specialized medical care and rehabilitation to assist with a full recovery from the virus.

The Post COVID Recovery Center at Franklin Cardiovascular Associates provides extensive evaluation and treatment services for those with lingering cardiovascular and pulmonary symptoms following COVID-19 infection. The Recovery Center provides the following Recovery Protocol available on their website. The Clinic is able to evaluate a patient with chronic fatigue and orthostatic intolerance symptoms with autonomic testing.

The Virtua Health Care After COVID Program provides web-based medical care for those with persistent symptoms following COVID-19 infection. Patients are first connected with an office assistant who will walk the patient through the process of registration and scheduling for their initial intake telehealth visit. The telehealth intake visit will then be conducted by a clinician, who will work with the patient to come up with the appropriate care pathway for their individual needs. The program offers a variety of specialist referrals.
