We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services). *Survivor Corps does not endorse any providers.

Multidisciplinary Long-Term Recovery
The University of Michigan Health Post-COVID-19 Clinic is administered through the Physical Medicine & Rehabilitation team at Michigan Medicine.
This program is designed to provide care in addition to care you receive from your primary care team or referring provider. The Clinic will be recommending self-management strategies, further evaluation, and/or medication options. Based on your symptoms, you may have up to three visits with a Physical Medicine & Rehabilitation provider, including an initial meeting and two follow-up appointments to identify areas of improvement. The goal is for you to continue follow-up care with your primary care team using the Clinic’s recommendations.
Depending on your symptoms, our recommendations may include physical, cognitive, and/or psychological treatment plans. One of the primary types of treatment we provide for patients with prolonged symptoms after COVID-19 are our Post-COVID Recovery Groups. These groups are led by our team of neuropsychological and mental health care providers and equip patients with education, skills, and interventions to address their current symptoms and optimize quality of life.
Over the course of 6 weeks (1 hour per week), participants will gain knowledge and awareness of factors that contribute to their post-COVID symptoms, from the evidence-based bio-psycho-social perspective. Patients will then be provided with clinical interventions and tools with the goal of learning and practicing behavior change, experiencing symptom reduction, and re-engagement in life.
Eligibility: Patients must meet several criteria to be seen at the Post COVID-19 Clinic:
Age 18 or older
A history of lab-confirmed COVID-19 at least 3 months prior to the referral
Continued symptoms post acute COVID-19 infection
Referral from a primary care provider (or inpatient provider at discharge)

Pediatric Post-COVID Syndrome Clinic at Michigan Medicine C.S. Mott Children’s Hospital

Pediatric Multidisciplinary Long-Term Recovery
Pediatricians affiliated with the Post-COVID Syndrome Clinic at Michigan Medicine C.S. Mott Children’s Hospital will assist children and families in coping with the long-term effects of COVID-19 which may include fatigue, shortness of breath, joint pain, chest pain, cough and loss of taste and smell. The Post-COVID Syndrome Clinic was established to address the needs of these patients and to help with recovery. The Clinic can also help assess and treat patients who have recovered from severe COVID or multisystem inflammatory syndrome in children (MIS-C).
Features of the Pediatric Post-COVID Syndrome Clinic include virtual and in-person visits, assessments of quality of life and ongoing symptoms, physical examination and treatment of symptoms, testing that may include pulmonary (lung) function testing, immune testing, chest imaging, or exercise testing, anticipatory guidance on COVID precautions and return to exercise, coordination of care with pediatric subspecialties such as pediatric pulmonology (lung), pediatric cardiology (heart), pediatric physical rehabilitation and medicine, pediatric psychology and pediatric otolaryngology when needed, coordination of care with primary care physicians, and opportunities to participate in research studies on the long-term effects of COVID-19 in children.
Patients at the Clinic must be under the age of 21, have a positive PCR test, have new or worsening symptoms that persist more than two months (60 days) after the initial COVID-19 infection, and have no other exposures or explanations for the new symptoms.

The Henry Ford Health COVID Recovery Care program is helping Henry Ford Health primary care providers treat patients with long-COVID symptoms. If your symptoms continue for more than 12 weeks, you may benefit from the combined knowledge of the experts that make up the COVID Recovery Care team. To make an appointment: At this time, the program is available only to established Henry Ford patients who have been referred by their primary care provider. Your Henry Ford primary care doctor can submit a consult through the electronic medical record to the COVID Recovery Care team. You will be asked to complete a symptom questionnaire to help providers better understand and track your symptoms. Your answers provide important information for the team to tailor and personalize your care plan.

Multidisciplinary Long-Term Recovery
Patients with a history of a positive COVID-19 test and lingering symptoms for more than one month can find specialized care at the Ascension St. John Center for Internal Medicine Post Acute COVID-19 Clinic.
Following initial evaluation, a team of specialists in Nephrology, Physical Medicine and Rehabilitation, Psychiatry, Neurology, Cardiology, Geriatric Medicine, Pulmonology and Internal Medicine will work to treat symptoms.

The DMC Rehabilitation Institute of Michigan (RIM) COVID-19 Rehab Recovery Program consists of breathing exercises and strength and endurance training to help patients who have recovered from COVID-19 return to their normal lives and activities. A prescription from your physician is needed to begin therapy with the Program.

Team Rehabilitation Post COVID-19 Recovery Program
Rehab and Physical Therapy
Team Rehabilitation has developed a Post COVID-19 Recovery Program to assist all patients affected by COVID-19 with their recovery and allow them to return to work and daily living. The Program is designed to help patients, using medically supervised therapy, so they may regain strength, respiratory function and resume normal lifestyle.
Throughout the therapy sessions, a patient’s heart rate, oxygen saturation and blood pressure are carefully monitored and that data is used to maintain a safe and effective recovery program.
Evaluation includes assessment of strength, flexibility, respiratory muscle function, and activity tolerance, using a Rate of Perceived Exertion scale. Standardized functional tests are performed while monitoring vital signs.
The program may include core, upper body and lower body strengthening exercises, a cardiovascular conditioning program, and stretching as indicated. Initially these sessions in the clinic may not be long, based on patient activity tolerance. The patient’s vital signs are monitored, and exercise is performed within target heart rates, while monitoring oxygen saturation levels.
It is important to string together frequent, short bouts of exercise early on to improve activity tolerance, which means the patient needs to be doing these activities multiple times per day. Patients will be provided with a home exercise program and a pulse oximeter for home use, and instructed how to safely monitor their own response to the exercises. As the patient grows stronger, the intensity and duration of exercise increases, for both strengthening and endurance. Supervised exercise time will increase.
If a patient is looking to return to work and is able to perform activities of daily living fairly well, they can be transitioned to a Work Reconditioning Program tailored to their needs, with input from their employer. At this point they will spend several hours in the clinic performing a variety of work simulation tasks, strengthening exercises, and cardiovascular activities.
The Post COVID-19 Recovery Program can be started in the home via TeleRehab and moved into the clinic at the appropriate time for more advanced activities. A patient’s therapy will be monitored closely in the home setting, via pulse oximeter device, and measured by a Physical Therapist. Our staff is fully qualified to anticipate potential over-stressing situations and will make adjustments to therapy during these monitored sessions to yield the best results for each patient.
