Every Post COVID Care facility offers different services and access to specialists. We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services). *Survivor Corps does not endorse any providers.

Multidisciplinary Long-Term Recovery

The Bristol Health Post-Acute Covid-19 Treatment and Rehabilitation (PACT) program is aimed at providing long-haulers with the individualized care they need to achieve maximum recovery.

PACT brings together a multidisciplinary team of experts from pulmonary and sleep medicine, cardiology, neurology, behavioral health, infectious disease, physical therapy, speech pathology and occupational medicine. Upon entrance into the program you will receive an initial symptom assessment to gain a better understanding of your needs.

Program participants are then referred to the appropriate specialists for further evaluation and management. For some, physical therapy may be the best route to a full recovery, for others it may take expertise from a neurologist to deal with cognitive issues, a cardiologist to address heart concerns or direct help from one or more of the many other specialists involved with this integrated program. All providers collaborate closely with the common goal of helping these long-haulers.

In addition to providing evidence-based care to aid in your physical recovery, PACT can also connect you with the proper resources to address any mental health issues brought on by the pandemic.

The Bristol Covid-19 Support Group provides another platform for these individuals to cope with long-term Covid effects.

Farmington

UConn Health Long-COVID Recovery Center

Multidisciplinary Long-Term Recovery

UConn Health has begun seeing patients in the Long-COVID Recovery Center to help patients dealing with their post-COVID symptoms. The Long COVID Recovery Center provides a centralized, comprehensive approach to address patients’ wide array of symptoms. Patients in the Long-COVID Recovery Center get a comprehensive evaluation by Dr. Ameer Rasheed, pulmonologist at UConn Health, who then coordinates the care with the patient to the appropriate specialists if needed.

Patients are referred by their primary care physicians at UConn Health after experiencing persistent symptoms beyond eight weeks.

Rehabilitation and Therapy

The Hartford Healthcare COVID Recovery Center has brought together a variety of medical specialists to treat individuals who are still experiencing COVID-19 related symptoms and to help them return to full health. The Center provides outpatient services including diagnostics, treatment, therapy and support groups. To treat symptoms such as muscle weakness, difficulty breathing, fatigue, balance issues, weakness or numbness in your arms and legs, difficulty walking or balancing, headache, memory issues, stroke, mental health issues, syncope, abnormal heart rhythms, palpitations or arrhythmia, and/or chest discomfort.

Patients enrolled at the Hartford Healthcare COVID Recovery Center must be Connecticut state residents with proof of a positive COVID-19 test.

Multidisciplinary Long-Term Recovery

The goal of the Center for COVID Recovery Care is to listen and partner with patients having symptoms or concerns weeks and months after their possible initial COVID-19 infection and support them through the recovery process, and connect them with available resources, including specialty care, if needed.

An initial telehealth visit will start with an assessment of the likelihood that you had COVID-19 in the past. The visit will include review of your current symptoms, tests done until now, and any previously completed treatments. Recommendations for further evaluation, next steps and or referral to relevant specialty care will be provided.

The Center for COVID Recovery Care welcomes patients whether or not they were tested in the past for the coronavirus.

Middlesex

Middlesex Health Post-COVID Rehabilitation

Rehabilitation and Therapy

Middlesex Health's team of physical, occupational and speech therapists can help those who are experiencing residual symptoms of COVID-19 even after no longer being contagious for the virus. Their multidisciplinary team creates individualized treatment plans that result in the best possible outcomes. Treatment options include:

  1. Endurance training

  2. Balance retraining

  3. Cognitive treatment focusing on "brain fog" (memory, concentration, confusion, etc.)

  4. Strengthening and customized exercise programs

  5. Activity modification education

  6. Pulmonary education

  7. Additional resources are also made available, including behavioral health services and nutritional counseling.

Both telehealth and in-person appointments are available for all Physical Rehabilitation services. Throughout the COVID-19 pandemic, Middlesex Health facilities are taking necessary precautions. Staff, patients and visitors are required to wear face masks, treatment areas and waiting rooms allow for social distance, areas are cleaned before and after each patient, and everyone entering a Middlesex Health facility has their temperature taken and is screened for other COVID-19 symptoms.

Motion PT Recovery Program

Each Motion PT location features a team of therapists who are trained in the Mount Sinai Post-COVID Care protocol and will help post-COVID patients safely get back to what moves them. [Make sure to ask for a therapist trained in COVID recovery]. Motion PT understands the need for care and compassion more than ever for patients suffering from Post-Acute COVID Syndrome (PACS). The PACS program caters to each patient’s individual needs, taking them through a supervised three-phase protocol.

The mission of Motion PT is to get each patient back to their pre-COVID self.The program aims to help patients recover from long term COVID symptoms such as Fatigue and weakness, Dizziness and confusion, Shortness of breath, Heart rate variability, Muscle/joint pain and swelling, and Headache.

Post COVID-19 Recovery Program at Yale Winchester Chest Clinic

Multidisciplinary Long-Term Recovery

The Post-COVID Recovery Program at the Winchester Chest Clinic provides comprehensive evaluation of persistent respiratory symptoms related to prior COVID-19 infection. These symptoms may include shortness of breath, cough, chest pain, or change in exercise tolerance. Initial visits include consultation with a pulmonary physician, full pulmonary function tests to assess lung performance, and functional assessment by the physical therapy team.

The Clinic also collaborates with other subspecialty physician experts across the institution to address other post-COVID symptoms and complications, including changes in memory/concentration, cardiac (heart) abnormalities, blood clotting disorders, and mood changes.

Dr. Jennifer Possick, MD is the Medical Director at Westchester Chest Clinic and brings a pulmonary focus to the program while partnering with caregivers from other specialties, including: Physical Therapy, Cardiology, Neurology, Psychiatry, Geriatrics and ENT.

Dr. Denyse Lutchmansingh, MD, is a Pulmonologist and Critical Care Specialist at Yale Medicine.

Pediatric Multidisciplinary Long-Term Recovery

Pediatric specialists at the Yale New Haven Children’s Post-COVID Comprehensive Care Program provide care to children who are experiencing residual, prolonged effects due to COVID-19 infection also known as long COVID / COVID-19 Syndrome. These effects can range from mild to moderate and may include:

  1. Inability to tolerate strenuous activity

  2. Anxiety

  3. Chest tightness

  4. Cognitive difficulties, also known as “brain fog”

  5. Extreme fatigue

  6. Headache

  7. Muscle pain

  8. Shortness of breath

  9. Palpitations

  10. Residual fever

Experts in infectious disease, pulmonology, cardiology, rheumatology and neurology see patients at the Pediatric Specialty Center in the Children’s Hospital, all during a single visit. Through this program, a tailored care plan is provided based on the latest research to aid each patient’s recovery. The care team also includes nurse coordinators, social workers and other interdisciplinary care team members who ensure each families’ needs are met.

In addition to addressing persistent symptoms after COVID-19, Yale New Haven Children’s Hospital offers programs involving return-to-sports clearance for children who had COVID-19 along with management of multi-system inflammatory syndrome (MIS-C).

Multidisciplinary Long-Term Recovery

WMCHealth is offering Post-COVID-19 care for ongoing recovery for patients in the Tri-State area, including CT, who continue to deal with issues surrounding their physical, mental and emotional health and quality of life. Patients with lingering symptoms after a known or suspected COVID-19 infection may make an appointment.

At your appointment with the Post-COVID-19 Recovery Program, a primary care doctor will give you a physical exam and review your COVID-19 history. Then, a clinical patient navigator will be assigned to guide you through a treatment plan tailored to your specific needs for specialized care.

The program provides patients with access to Westchester Medical Center physicians with expertise in primary care, pulmonology (lungs), cardiology (heart), nephrology (kidneys), psychiatry (mental and emotional health), radiology, and other specialties as needed. WMCHealth physicians who evaluate you will follow up with your regular doctor to coordinate your ongoing treatment and recovery plan. Multidisciplinary Long-Term Recovery

Stamford Health’s Center for Integrative Medicine & Wellness is focused on helping you regain your strength after COVID-19 infection with evidence-based treatment options that fit your lifestyle. Stamford Health’s group of physicians will customize a COVID-19 recovery plan that works for you and respects your needs, preferences and values.When you come in for treatment, the team will ask about your lifestyle before COVID so they can tailor your treatment plan to you. Your doctor at the Center for Integrative Medicine and Wellness will explain the Program’s evidence-based approach which may consist of any or all of the following: 

  1. Pain management options without the use of opioid drugs.

  2. Mind-body stress reduction - encouraging self-care such as breathing work, muscle relaxation and guided imagery to reduce anxiety. 

  3. Diet management—focusing on the benefits of consuming anti-inflammatory foods as based on the Mediterranean diet. The Program will advise you on foods to avoid and any dietary supplements to incorporate into your daily routine.

  1. An initial assessment over the phone or in-person, depending on your comfort level. At this appointment, the Program will create a treatment plan for you.

  2. A follow-up appointment each week for four weeks where the Program team will discuss how you have been feeling, and reassess the need for any additional treatment going forward.

Eligibility: If you are no longer infected with the COVID-19 virus but still have ongoing symptoms, the COVID-19 Recovery Program will be happy to schedule an initial health assessment for you.

Acute COVID-19 Recovery + Rehab and Physical Therapy

Gaylord treats patients recovering from COVID-19 in inpatient and outpatient settings. Inpatients come to Gaylord from an acute care hospital may need complex medical care, pulmonary care, ventilator weaning, and intensive rehabilitation. After recovery from initial COVID-19 infection, the outpatient team works with patients to coordinate care while prioritizing the patient's personal recovery goals.

Common residual effects of COVID-19 addressed through outpatient services may include breathing difficulty and shortness of breath, muscle weakness, neuropathy and achy joints, depression and anxiety, cognitive processing and executive functioning difficulties - short term memory loss - “brain fog.”
