Every Post COVID Care facility offers different services and access to specialists. We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services). *Survivor Corps does not endorse any providers.

Acute COVID-19 Recovery

In order to properly support COVID-19 patients who required COVID-related intensive care, especially those off ventilator support, UCHealth is now offering a Post-Intensive Care Clinic on the Anschutz Medical Campus.

Dr. Sarah, Jolley, MD leads the PIC Clinic’s multidisciplinary team of providers with support from Pulmonology, Physical, Occupational and Speech Therapy, Pulmonary Rehabilitation, Physical Medicine and Rehabilitation and Psychiatry.

Multidisciplinary Long-Term Recovery

Colorado COVID Care is an Integrative Medicine clinic for long COVID patients located in Boulder, Colorado. Multidisciplinary Long-Term Recovery/Rehabilitation and Therapy

The Family Health West Post-COVID Recovery Team is comprised of a team of Pain Specialists, Neurologists, Respiratory Therapists, Occupational Therapists, Physical Therapists, Speech Pathologists, Psychologists, Exercise Physiologists and Physiatrists to combat adverse long-term health effects of the coronavirus. The Team is prepared to address a wide range of long-COVID symptoms, such as fatigue, brain fog, heart palpitations, decreases in lung function, memory loss, and depression. Each patient will receive a full physical and mental health assessment from the Recovery Team before being paired with specialists equipped to meet their individual needs.

Telehealth and outpatient care options are offered. No referral required, but the patient must have a primary care provider or be willing to receive one.

Neurological Long-Term Recovery / Rehab and Physical Therapy

The Rehab Without Walls NeuroSolutions Post COVID / PICS Program is a coordinated interdisciplinary approach that offers recovery services for people with long-lasting effects of COVID and for those that have spent significant time in an acute care setting.

The Program's combined team expertise provides a holistic and comprehensive approach to care. All members of the team consistently address physical and cognitive challenges to improve self-care and return to functional independence.

The Post-COVID Program offers a home and community program which blends evidence-based therapies with creative uses of the patient’s own surroundings (at home, school, work, community). The goal for the patient is to recover, learn and grow – in comfortable and familiar places. This leads to rapid and lasting functional independence and patient success. Family members are encouraged to participate for maximum benefits.

The goals for each patient of the program are to:

Multidisciplinary Long-Term Recovery

Long COVID NP is a virtual clinic that uses an integrative medicine approach, which combines practices of both conventional medicine and functional medicine.

The clinic will discuss in-depth things like lifestyle, diet, exercise practices, and previous records with each patient to formulate a plan that may include recommendations, referrals, supplements, and prescription medications.

Eligibility: Long COVID NP accepts all patients who are suffering from long COVID weeks to months after their initial COVID diagnosis.
